You will receive an email 3 days prior to your in-person visit (or 1 day prior to your telehealth visit) with a personalized link to set up an account. You will then be able to complete intake questionnaires and get ready for your visit. When you are next seen in person at one of our sites, we will be able to verify your identify in person and enable you for full system access.
